Position Summary

We are currently looking for Supported Living Facilitators to join our Team initially on a casual basis within Dalby, Toowoomba and Gatton. The right people will have the opportunity to progress from casual to contracted positions.

The Role

As a Supported Living Facilitator you can make a difference to the lives of the people we support under the NDIS. You will work with people who have varying abilities who may be impacted by physical and/or psychological disabilities to help ensure they are able to maintain quality of life.

The support we provide varies this could include but not be limited to helping the people we support in their own homes or within group care homes assisting them  with tasks and activities such as day to day household and/or personal tasks (such as bathing)which could include overnight stays, getting out for a coffee or catch-up with friends, participating in a hobby or sporting activity, going on  day trips or holidays, visiting medical appointments and any other support that may contribute to helping them achieve their goals while  staying healthy, happy and safe.

About you

You will be an active member of your local community, have passion, energy and a background that has seen you working or volunteering within a caring capacity role previously supporting people within all community groups including (ect Aged, Disability or Children & Youth). Although not mandatory a qualification within community or human services would be desirable.

Before commencing with UCQ you will also need to provide a Senior First Aid Certificate, Drivers License and currently hold or be willing to obtain positive notice from Disability Services (Yellow Card) and Working with Children Check (Blue card).
