Uniting Care Better Futures are currently looking for a motivated person who is passionate about facilitating opportunities alongside people with disabilities to join our team as a Lead Practitioner Community on a full time basis Located in our Aitkenvale, Townsville office.

 The Role:  

In this role you will act as the lead disability practitioner at a service level providing guidance and leadership to employees delivering disability services. In addition you will work closely with individuals and their families throughout the NDIS planning process to agree on support plans and convert those plans into service agreements and service bookings to allow individualised billing consistent with NDIS pricing schedules.

 About You:

You will have excellent leadership capabilities with the ability to manage multiple priorities and stakeholders, along with highly developed interpersonal communication skills to assist you in building rapport with the person we support, their family and their support teams. In addition you will have experience with service delivery and redesign at an individual and group level preferably within disability.

 Supplementing the above, you will hold at least a Certificate 3 level qualification in Community Services or related field, intermediate systems experience (including MS Office and Care Management Systems) and a current Qld Driver Licence. You will also hold a current Working with Children Check (Blue Card) and a positive notice for working with people with a disability (Yellow Card), or have the ability to apply for these.
