UnitingCare is an innovator and market leader in the disability and aged care services. We are registered as a Support Coordination service under the National Disability Insurance Scheme (NDIS).
We are looking for a Support Coordinator who has a passion to assist NDIS participants to implement their NDIS plans in accordance with their goals and desired outcomes. The Support Coordinator will facilitate their choice and control in accordance with the principles of the NDIS.
What do you need to be successful in this role?
- A strong understanding of the NDIS and the purpose of support coordination
- Have completed a relevant tertiary qualification or appropriate disability specific qualification (Cert 4 or above) and have at least five years’ experience in a relevant human services sector
- Experience in coordinating, or assisting in the coordination of community based services, and/or a background in community development
- An understanding of person centered practice and contemporary approaches to service design
- A strong commitment to people with disabilities living full and meaningful lives
- Well developed decision making and analytical skills
- Good networking skills and the ability to access community opportunities for the person they are supporting
- Effective interpersonal and written communication skills including experience in enabling others to set goals and build capability and confidence
- Proven organisational skills, ability to multi-task and prioritise workloads
- Ability to process information and problem solve to reach a defined outcome, particularly in the human services context and meet deadlines
- Criminal history checks for Blue and Yellow Cards, or the ability to obtain them
- A current Queensland Driver's Licence
