Position Summary

We are currently looking for a motivated person who is passionate about facilitating opportunities alongside people with disabilities to join our team as a Lead Practitioner - Supported Living on a Permanent-Part Time basis of 28hrs or more per week, Located in Townsville.

The Role:

The Lead Practitioner’s role is to deliver sustainable high quality support to people with a disability in both home and community life so that they can achieve personal goals and maintain socially valued roles, friendships and relationships, within the resources available to them. These resources include funding, family, informal supports and community resources.

This particular role has been created to mentor a small team of Supported Living Facilitators to assist a individual with a disability to achieve their personal goals & outcomes as well as collaborating with their informal supports and community networks. Some   community involvement may include attending local music events, shopping for her groceries, or trips to local clothing shops. This person also enjoys being with people, dogs, birds, music, dancing and developing social identity within the local community. This role plays a crucial part in exploring what other opportunities may be possible for this individual.

About You:

We are looking for a positive, energetic, enthusiastic person who has the skills and creativity to explore home & community opportunities with this individual and the team of Supported Living Facilitators. You will have excellent leadership capabilities with the ability to manage multiple priorities and stakeholders, along with excellent communication skills to assist you in building rapport with the person we support, their family and their support teams. In addition you will have experience with service delivery at an individual and group level.

Supplementing the above, you will have intermediate experience in Community Services or related field, systems experience (including MS Office and Care Management Systems) and a current Qld Driver Licence. You will also hold a current Working with Children Check (Blue Card) and a positive notice for working with people with a disability (Yellow Card), or have the ability to apply for these.
