An excellent opportunity has arisen to become a member of our high achieving, client centred team with the Transition Care Program (TCP). The Transition Care Program is seeking a dynamic and experienced health professional from an allied health or nursing background to fulfil the role as TCP Clinical Coordinator.

The Transition Care Clinical Coordinator is responsible for the leadership and management of patient flow into and out of Transition Care, ensuring quality patient care as well as providing leadership and support to the team of dedicated care coordinators. 


Through your knowledge you will have a leading role in ensuring excellence in patient care, promotion of quality improvement, education of staff and service development. This position is primarily based at Bundoora Extended Care Centre, however the role includes providing patient assessment and staff guidance at other Northern Health campuses.
