CASE MANAGER/PLANNER

$59,092 to $80,698 (Full Time rate) + superannuation + salary packaging based on experience and qualifications

The Department and Team

The Community Care Division of GV Health provide a range of aged and disability supports across programs including Home Care, NDIS Support Coordination, CHSP Respite and Early Childhood services and supports


The role

We are seeking several Case Manager/Planners

To be successful in the role you will need:

  • Tertiary qualification in health and community services and/or business management
  • Have a working knowledge in the aged and disability sector and an understanding of case management and how it relates to working in a regional setting
  • An understanding of NDIS

Selection Criteria Essential: Commitment to the GV Health Values of Compassion, Respect, Excellence, Accountability, Teamwork and Ethical Behaviour.
Essential: A Tertiary qualification in an appropriate Health and/or Human Services field, or extensive, relevant experience
Essential: Sound experience of the knowledge and skills required in working with frail older people, people with disabilities and their carers
Essential: Well-developed interpersonal and communication skills, including the ability to initiate and maintain contacts with a broad range of service providers and community agencies
Essential: Demonstrated ability to work independently
Essential: Knowledge of the Case Management approach to service delivery, in a rural community setting
Essential: Satisfactory National History Criminal Check, satisfactory Victorian ‘Employee’ Working with Children Check and completion of a Commonwealth of Australia Statutory Declaration prior to commencement of employment.
