Position Summary

Case Manager / Planner

$68,478 to $80,698 (Full Time rate) + superannuation + salary packaging based on experience and qualifications

The Department and Team
The Community Care Division of GV Health provide a range of aged and disability supports across programs including Home Care, NDIS Support Coordination, CHSP Respite and Early Childhood services and supports.

The role
To be successful in the role you will need:

  • Tertiary qualification in an appropriate Health and/or Human Services field, or extensive experience
  • Have a working knowledge of the aged and disability sector and an understanding of case management and how it relates to working in a regional setting
  • An understanding of NDIS

Selection Criteria Essential: Demonstrated knowledge, skills and experience working with frail older people, people with a disability and their carers
Essential: High level interpersonal and communication skills, and the ability to establish good working relationships with a wide range of services
Essential: The ability to display sound independent judgement and innovative problem solving skills

Essential: A satisfactory National Criminal History Record Check, Victorian 'Employee' Working with Children Check, Disability Workers Exclusion Scheme List Check and completion of a Commonwealth of Australia Statutory Declaration prior to commencement of employment
