The Integrated Care Services Division offers a diverse range of programs and services which provide high quality person centred care to patients, clients, residents and consumers throughout Bendigo and the Loddon Mallee Region.  Services are provided in inpatient, outpatient, community and residential care settings including at the Bendigo Hospital site, a number of sites in Bendigo and across the Region and in client’s homes.

Our services include one of largest public community dental services in Victoria, allied health, a comprehensive palliative care service, rehabilitation and geriatric evaluation, residential care, community care packages, carer support services, community nursing, aged care assessment, transition care, residential in reach, hospital admission risk program, post-acute care, diabetes education, aboriginal services, health promotion, diversity and interpreting services, pastoral care and a large team of volunteers.

 

The Community Care Services Team

Community Care Services which is part of the Bendigo Health Care Group (BHCG) provide a community care workforce to the local community. Services are provided to consumers of Bendigo Health programs – Post Acute Care, Transition Care Program, Home Care Packages, Palliative Care and Carer Support Services. In addition, services are provided to people eligible for the Home and Community Care Program for Younger People (HACC PYP).

Salary:               $21.35 - $30.35 per hour + over award
Status:               Casual roles (flexibility required)

Selection Criteria
Essential:
01.Accredited Certificate III in any one of Aged Care Work, Home and Community Services or disability or equivalent - and a willingness to maintain or upgrade training as required.

Essential:
02.Own motor vehicle including valid registration and full comprehensive insurance including replacement vehicle insurance and valid driver’s licence.

Essential:
03.Ability to provide community based care and form supportive relationships with consumers.

Essential:
04.Ability to work with people with dementia and/or challenging behaviours.

Essential:
05.Ability to work independently and within the care plan.

Essential:
06.Ability to identify and report any relevant changes to coordinator.

07.Ability to seek support when required.

Essential:
08.Excellent written and verbal communication skills.

09.Satisfactory completion of a pre-employment fit for role assessment.
