THE INTEGRATED CARE SERVICES DIVISION

The Integrated Care Services Division offers a diverse range of programs and services which provide high quality person centred care to patients, clients, residents and consumers in Bendigo and across the Loddon Mallee Region.  Services are provided in inpatient, outpatient, community, home and residential care settings.

 

THE COMMUNITY CARE SERVICES TEAM

Community Care Services which is part of the Bendigo Health Care Group (BHCG) provide a community care workforce to the community in the Shire of Campaspe. Services are provided to consumers of Bendigo Health programs – Post Acute Care, Transition Care Program, Home Care Packages, Palliative Care and Carer Support Services.

 

THE POSITION

The Community Care Worker provides a valued and key role in delivering high quality, professional, comprehensive and integrated support services for people receiving support in the community.

The role includes promoting safety and independence, enabling people to remain in their homes and to engage with their communities.  Services will be delivered according to the care plan. A care plan refers to the document itemising activities to be undertaken with an individual consumer.  Including (but not limited to):

 

  • Short term support for people returning home following an admission to hospital
  • Ongoing support services in the community for people who receive a Home Care Package
  • Support services for people in the community receiving palliative care
  • Respite services that are tailored to meet the needs, interests and leisure activities of the consumer
  • Undertaking a re-ablement approach to all tasks undertaken
  • Working flexible shifts including split shifts, evenings and weekends

The position requires that the worker maintains a vehicle for employment purposes that is:

  • Roadworthy
  • Comprehensively insured
  • Has an insurance statement listing use for BH business purpose and with hire car options
  • Accepts the vehicle is subject to a minimum annual safety check by Bendigo Health
  • Accepts the vehicle is subject to referral for a roadworthy certificate, if issues are identified at the safety check
  • Accepts the cost to ensure the vehicle is roadworthy
  • Notifies Bendigo Health before using any other vehicle for work purposes

Selection Criteria
Essential:
01.Accredited Certificate III in any one of Aged Care Work, Home and Community Services or disability or equivalent - and a willingness to maintain or upgrade training as required

Essential:
02.Satisfactory completion of a pre-employment fit for role assessment

Essential:
03.Own motor vehicle including valid registration and full comprehensive insurance including replacement vehicle insurance and valid driver’s licence

Desirable:
04.Ability to provide community based care and form supportive relationships with consumers

Desirable:
05.Ability to work with people with dementia and/or challenging behaviours

Desirable:
06.Ability to work independently and within the care plan

Desirable:
07.Ability to identify and report any relevant changes to coordinator

09.Excellent written and verbal communication skills
