An exciting opportunity has emerged for a qualified enthusiastic and motivated Social Worker with an interest in the management of clients with complex psychosocial issues to work in the Hospital Admission Risk Program (HARP).  HARP provides integrated and coordinated care to clients with a chronic disease, aged and/or complex needs who frequently use hospitals or are at risk of hospitalisation.

Selection Criteria
Essential:
01.AASW accredited Bachelor of Social Work or equivalent and eligibility for accreditation status with the AASW

Essential:
02.Capacity to provide a range of high standard interventions including individual and family counselling, crisis intervention, short term and supportive counselling, group work, education, advocacy and health promotion as appropriate to the needs of the patient and/or family

Essential:
03.Demonstrated knowledge and commitment to best practice guidelines in the care of clients with complex needs

Essential:
04.Demonstrated knowledge of specialist programs to ensure the appropriate and timely provision of information and services is given to patients/ carers

Essential:
05.Demonstrated expertise in working in a multi-disciplinary team

Essential:
06.Demonstrated time management skills to manage a caseload, meet deadlines, maintain schedules, set goals/objectives, as required

Desirable:
07.Experience in the supervision of undergraduate students
